The following information is intended for students who are eligible for OSAP assistance and who are studying via Distance Education or studying at a University of Windsor Satellite Campus.
Once your OSAP eligibility has been confirmed and if you will need your OSAP funding to be mailed to you, you must complete the following form(s) and mail or fax it to the Student Awards & Financial Aid Office:
Together with this form(s), you will also need to send a copy of your Social Insurance Card and a copy of your Government Issued photo ID. Be sure the the copies are legible.
Once our office receives your request with your identification, we will mail you your Confirmation of Enrolment and your funding certificate (OSAP) with instructions on what you will need to do. Please read these instructions very carefully as failure to follow the instructions will cause delays in you receiving your funding. You will need to mail your completed and signed Confirmation of Enrolment back to our office (as per the instructions) so that you're second installment of funding can be requested from the government. If you are studying and receiving OSAP funding for one term, the second installment will be mailed to you within one month of you returning your Confirmation of Enrolment. If you are studying and receiving OSAP funding for two terms (Fall and Winter), your second installment of funding will be mailed to you once classes start in January.
Unless otherwise advised, arrangements will be made for your OSAP funding to be sent to the Financial Aid Office at the University of Windsor Satellite Campus where you will be instructed to pick up your funding from.
