The following forms can be used to make changes to your OSAP application. Please download, complete and submit to the Student Awards & Financial Aid Office. Forms must reach the Student Awards Office within 8 weeks from the end of the study period. Additional documentation and requests for changes may not result in additional funding if the requests are received after this period. OSAP funding will not be issued to students if it is within 21 days from the end of the study period. The Student Awards & Financial Aid Office requires approximately 2 weeks to process changes.

How to Process Your OSAP Funding Certificate

| Intersession & Summer Extension Form (2013) |
This form is used by students who have an OSAP account for the 2012/2013 academic year and who have already received OSAP funding from the University of Windsor for the current academic year. |
| Intersession & Summer Information & Fact Sheet | This information/fact sheet will provide you with information related to requesting OSAP funding for the intersession and/or summer including course load requirements (very important!). Once a student is assessed for OSAP funding for the intersession and/or summer session, before ANY course load adjustments are made or decisions made to drop courses, students should first consult with the Student Awards & Financial Aid Office to determine if there will be an affect on the OSAP assessment. |

| Adding a Dependent Child | This form is used by students who have had a child after they have submitted their OSAP application. Note, that if a student was a single student, has a child and is now a sole support parent, there are additional status change forms that must be submitted as well. See below. |
| Affidavit | This form is used by students who must provide documentation as to their status i.e. separated, sole-support parent. This document must be notarized by a Commissioner of Oathes. |
| Changing Your Status from Single to Sole Support Parent or Single Independent - MTCU Form | This form is used by students who were single dependent/independent when they applied for OSAP funding for the current academic year, and have since had a child. The child must be residing with the student full-time during the study period to be considered a sole-support parent. This form can also be used for students who are married, and their status has changed to separated, divorced or widowed with no dependent children residing with them. |
| Changing Your Status from Single to Married (2A) - MTCU Form | This form is used by students who were single depenent/independent when they applied for OSAP funding for the current academic year, and have since been married. Students must also submit form (2B) and (2C) with this status change request. |
| Spousal Information/Updates (2B) - MTCU Form | This form is to be used together with form (2A) above in order to update the spousal information for the purposes of the status change request. |
| Spousal Signature Pages (2C) - MTCU Form | This form is to be used together with form (2A) and (2B) above. The spouse is required to sign the forms and return with the package. This form can only be used with a status change request and can not be used for an original web application submission. |
| Changing Your Status from Single Independent to Single Dependent Student | This form is to be used when a student incorrectly applies as a Single Independent Student to change their status to Single Dependent. This form must be submitted with form (3B). |
| Parental Signature Pages - MTCU Form | This form is to be used together with (3A) to complete the status change request from Single Independent Student to Single Dependent Student. This form can only be used with a status change request and can not be used for original web application submission. |

| History of Canadian Residency for Student - MTCU Form | This form is to be used by a single independent student to clarify their residency history in Canada in order to determine whether or not the student meets the Ontario Residency Requirement. |
| History of Canadian Residency for Student and Parents - MTCU Form | This form is to be used by a single dependent students and their parents to clarify their residency history in Canada in order to determine whether or not the student or parents meet the Ontario Residency Requirement. |
| History of Canadian Residency for Student and Spouse - MTCU Form | This form is to be used by a students who are married and their spouse to clarify their residency history in Canada in order to determine whether or not the student meets the Ontario Residency Requirement. |

| Important Note: | Dropping your course load below 60% of a full course load (40% for students with a permanent disability) will affect your OSAP entitlement. You will lose weeks of eligibility (reassessed and overpaid) and you will be placed on OSAP Academic Probation or Restriction depending on the number of instances on your file). |
| Course Load Update - Fall/Winter | This form is used by a student who must advise of a course load change from the original course load that they were assessed for. NOTE: Changing your course load from 5 to 4 courses does not require you to submit a change form. If you are advising our office that you will not be returning for the Winter term or if you are changing your application to a Winter term only assessment, you MUST also submit an Income Update Form to ensure that your income is updated according to your revised study period. If you are dropping below 60% of a full course load (40% for students with a permanent disability, and you will be returning for the Winter term, you should complete form (4A - Withdrawn and Reinstated Form). |
| Course Load Update - Intersession/Summer | This form is used by a student who must advise of a course load change from the original course load that they were assessed for. Be careful as Intersession and Summer Session can get tricky with respect to OSAP eligibility. Be sure that you have read the Intersession & Summer Information and Fact Sheet and if you are unsure, please contact our office. |
| Withdrawn and Reinstated Form (4A) | This form is used by a student who has withdrawn from the current term but will be returning in the next term. For example, a student has withdrawn or dropped below 60% (40% for a student with a permanent disability) and who intends to register again in the Winter term taking a minimum of 60% of a full course load. |
| Income Update Form - Fall/Winter | This form is used by a student who wishes to update their income and resources in any of the income reporting periods on their OSAP application. Students are not required to report work study or institutional bursary funding unless otherwise notified by email or in writing. Student who are required to update their file with scholarship or bursary income should use form (4B) |
| Income Update Form - Intersession/Summer | This form is used by a student who wishes to update their income and resources received during the summer and who has received OSAP funding in the summer term. |
| Award Declaration Form (4B) | This form is used by a student who must update their OSAP file with award income that they have received. Students are notified when they must report the award. Not all bursaries require reporting as many need based awards are reported directly to the government in a manner that does not affect a student's OSAP funding. |
| Clarification of Work & Study Terms for Co-op Students | This form is used by a co-op student who must clarify their work and study sequence for the current academic year. |

| Important Note: | Students studying on a Letter of Permission at a post-secondary institution outside of Canada can not have their costs added to their OSAP assessment at the University of Windsor. If you are planning on taking courses at Athebasca University on Letter of Permission, we strongly recommend that you come to our office at least 3 months in advance of your intended start date to ensure that you have the correct information as to how to proceed with OSAP funding. |
| Students Studying in a Collaborative or Concurrent Program | This form is used by students who are studying in a collaborative or concurrent program with a community college i.e. Nursing or BABEdECE. Often, students are registered full-time (taking 60% or more of a full course load, 40% for a student with a permanent disability) at one institution but they may be taking 1 or 2 courses at the other institution. We can combine the costs of both institututions provided that the costs at the other institution are specific to the collaborative program. |
| Students Studying at Another Institution on a Letter of Permission |
This form is used by students who are registered full-time (taking 60% or more of a full course load, 40% for a student with a permanent disability) at Windsor but they may be taking 1 or 2 courses at the other institution on a letter of permission. We can combine the costs of both institututions provided that the courses being taken at the other institution will be transferred back to their program at Windsor. |

| Confirmation of Permanent or Temporary Disability Form | This form is used by students who must confirm their permanent or temporary disability as required by the OSAP program in order to qualify for certain grants and buraries and to allow a student to take 40% of a full course load to be considered for funding under the full-time program. |
| Confirmation of Permanent Disability of Child Form | This form is used by students who must confirm that their dependent child has a permanent disability. |
| Continuation of Interest-Free Status Form - MTCU Form | This form is used by student who must confirm their full-time status for the purposes of maintaining their loans in interest-free status. This form must be completed WITHIN 6 months from the end of their last full-time study period. For example, if a student was last a full-time student in April, then they must confirm enrolment (submit this form to the Student Awards & Financial Aid Office) by October 15 to ensure that the file is updated PRIOR to November 1. Otherwise, the student will go into repayment and interest for the 6 month period will be added to the principle of the loan. | Grade Confirmation Form | This form is used by student who are on OSAP Academic Probation and who must be able to demonstrate that they have successfully completed 60% of a full course load (40% for students with a permanent disability) in their previous semester before they can receive funding for the next semester. Often, grades are not on SIS yet at the time that OSAP funding is available. If marks are unofficial and have been posted by the department, students can submit this form in advance of the grade being posted on SIS. |
| ODSP/OW Caseworker Letter | This form is to be used by students who are in receipt of ODSP/OW benefits. Because ODSP/OW funds a student's living expenses, a student can not receive a living allowance through OSAP as well otherwise an ODSP/OW overpayment will occur and further challenges could result. This form is used to confirm that a student who is in receipt of ODSP/OW is only eligible for direct educational costs (tuition, fees, books, travel allowance) and not eligible for additional or top up funding from OSAP. |
| OSAP Fee Redirection Authorization Form | This form is to update the authorization to redirect or remit fees from your OSAP funding to the University of Windsor. If you selected to opt out of fee redirection and you have now changed your mind and will be unable to pay your fees out of your own resources, use this form to opt in. If you had selected to opt into fee redirection and now you have changed your mind, use this form to opt out. If you are opting out, please be sure that your tuition fees are paid by the posted deadline date. |
| Request to Have OSAP Funding Certificate Mailed | This form is to be used by students who are studying via Distance Education or via a Satellite Campus ONLY and are not able to come to Windsor to claim their funding. Students must provide identification as part of their request and as outlined on the form. If a students wishes to have their funding couriered to them, they must provide a pre-paid Express Post envelope. Otherwise, documents are mailed regular mail. We will only mail in Canada and in some cases, to the US. Otherwise, students should arrange for a Power of Attorney. Contact the Student Awards Office for more details. |
| Request for OSAP Access Number Form - MTCU Form | The Request for OSAP Access Number (OAN) form is for students who are unable to access the secure side of the OSAP website because they have forgotten or don’t know their OSAP Access Number (OAN). We strongly recommend that you try to log into the “Forgot Your OAN?” feature on the OSAP website. You can also retrieve your OAN in person by simply going to the nearest financial aid office at any provincially funded university or college of applied arts and technology in Ontario. You MUST have your Social Insurance Card and valid government (federal or provincially)issued photo ID with you. |
| Request for OSAP Temporary Password - MTCU Form | The Request for a Temporary OSAP Password form is for students to request a temporary OSAP password. Use it if you are unable to access the secure side of the OSAP website because you have forgotten or don’t know your password, or are locked out of the website on either a temporary or permanent basis. We strongly recommend that you try to log into the OSAP Website using your challenge questions. You can also reset your password in person by simply going to the nearest financial aid office at any provincially funded university or college of applied arts and technology in Ontario. You MUST have your Social Insurance Card and valid government (federal or provincially)issued photo ID with you. |
| Student Assessment Summary Request | This form is used to request a manual assessment for the purposes of providing a government agency or another third party with your OSAP information and details. These requests will only be accommodated if the matter is of an urgent nature so you must be prepared to identify/document the nature of the extenuating circumstances. Otherwise, our office will order a Student Assessment Summary for you which will be issued and mailed to you by the Student Financial Assistance Branch and received generally within 7-10 business days. |
| Student Budget Form | This form can be used by students to establish a budget for the acaedmic year that they are applying for OSAP for. It can also be used to demonstrate expenses for the purposes of submitting a request for a review for OSAP funding or bursary assistance. |
| Supporting Documentation Cover Sheet | This form can be used to attach to your supporting documentation when submitting information/supporting documentation to our office for further review. |
| Tuition Fee Deferral and OSAP Form - See Student Awards & Financial Aid | This form can be used by students who have a tuition balance owing to the University and who can not register as a result provided that the student has forthcoming OSAP funding that is sufficient to cover the balance owing in addition to what will be owed if allowed to register. You must inquire first with Student Awards & Financial Aid to determine whether or not you are eligible. |
| Proof of Parental Foreign Income | This form provides students with the information required to document parental foreign income for the purposes of receiving the Ontario Tuition Grant or the Ontario Student Opportunity Grant where the parental income can not be verfied as the parent(s) does not file a Canadian Income Tax Return. This form can be used as a cover memo to the documentation that is being submitted to ensure efficient processing. |
