
In order to help speed up the process of your registration through the Course Change Form, here is what you must do:
1. Ensure that you are using the Engineering course change form.
2. Completely fill in the personal information at the top (Student ID Number, Family Name, First Name, Local Address, Postal Code, Major, Email and Phone Number). Also make sure that you sign and date the form and check off the appropriate box indicating the reason you are using the course change form.
3. When you fill in the Courses to be Added make sure that for every course you are adding you also register for the required lab section and fill in the Exam Slot information.
4. For each Course Change Form you also need to submit the following:
i) a letter explaining why you are registering with this form and why you cannot register online and if it is not a required course, you must explain in your letter how you want the course to be shown on your degree audit.
ii) a printout of your transcript from the SIS
Please pay attention to the registration deadline for each semester as it may take up to three days to have your paperwork signed by the Department Head.
Once your form and supporting documents are submitted to the Departmental Office the process of approval is as follows:
i) your form is reviewed by both the Undergraduate Advisory Committee and the Department Head;
ii) once the form is signed by the Department Head you must pick it up from the Departmental Office and bring the form with all required documents to the Professor for his signature;
iii) once your professor(s) have signed the form, bring the form, with all required attachments, to the Associate Deans Office for his final approval; and
iv) once the Associate Dean approves your request you must bring your form to Registrars.
Failure to submit an add/drop form that is complete with all the required attachments will result in delays in your registration!
