Downloadable Memo for Green Shield Benefits, Winter 2013 Coverage Period and Opt-In/Opt-Out Information 

This memo applies to new incoming graduate students for the winter 2013 semester.

THE OPT-OUT ONLINE DATES ARE JANUARY 7, 2013 to midnight Febuary 3, 2013 AT MIDNIGHT.  After this, the website will be closed.

If you are a part-time graduate student and wish to opt-in to the GSS benefits plan, you may do so at the GSS office only from January 7, 2013 to Friday February 1, 2013 by 12:30 p.m.  You are not assessed the fee by the University of Windsor for the GSS benefits plan, (check your student account), nor are you required to pay this fee and that is why you do not have coverage as a part-time graduate student.  However, if you would like to have 8 months of benefits coverage you may opt-in by completing the application form and paying by cheque made payable to the GSS or exact cash only in the amount of $375.95 for single coverage.

If you wish to opt-in your spouse, you may do so at the GSS office during the OPT-IN/OUT deadline of January 7, 2013 to February 1, 2013 by 12:30 p.m. and please bring the completed opt-in form which is also located on the GSS website. The cost to include your spouse for 8 months of coverage is an additional $342.44.  However, if you are a full-time graduate student and were charged for single coverage for the GSS benefits plan for the fall 2012 semester BUT DID NOT INCLUDE YOUR SPOUSE DURING THE OPT-IN PERIOD, YOU WILL BE CHARGED THE FULL AMOUNT BACK TO THE FALL 2012 SEMESTER FOR COUPLE COVERAGE IN THE AMOUNT OF $517.05 AND AN ADDITIONAL PENALTY OF $25.

If you wish to opt-in your family members you may do so at the GSS office during the OPT-IN/OUT deadline of January 7, 2013 to February 1, 2013 by 12:30 p.m. and please bring the completed opt-in form which is also located on the GSS website.  The cost to include your family members (which includes your spouse and up to 4 children) for 8 months of coverage, there is an additional fee of $558.44.  However, if you are a full-time graduate student and were charged for single coverage for the GSS benefits plan for the fall 2011 semester BUT DID NOT INCLUDE YOUR FAMILY MEMBERS DURING THE OPT-IN PERIOD, YOU WILL BE CHARGED BACK TO THE FALL 2012 SEMESTER FOR FAMILY COVERAGE IN THE AMOUNT OF $841.05 AND AN ADDITIONAL PENALTY OF $25.

The final date for opting-in is Friday, February 1, 2013 by 12:30 p.m.  The final date for opting-out online is midnight February 3, 2013.  After this date, the availability for online opting-out will be permanently closed.  The GSS hours of operation are Monday to Thursday 8:00 a.m. to 12:00 p.m. and 1:00 p.m. to 3:30 p.m. and we are closed for lunch from 12:00 p.m. to 1:00 p.m. during these days.  On Friday, the GSS hours of operation are 8:00 a.m. to 1:00 p.m. only.

If you opted-out during the fall 2012 semester as a full-time graduate student, DO NOT opt-out again AS YOU ARE NOT CHARGED FOR THE GSS BENEFITS PLAN AND ARE NOT ENTITLED TO A REFUND CHEQUE AGAIN.  If you have another comparable benefits plan, you are eligible to opt-out through the GSS website and follow the instructions.  Fill in the online application form.  Once you have completed the online opt-out application form, you will receive an automated receipt by email that you have successfully opted-out.  Please make sure to keep your online opt-out email receipt.
