

2011-2012 Windsor Program Awards of Excellence
The Associate Dean, Windsor Program solicits nominations for three newly created awards. These awards are the most prestigious awards given by the Windsor Program in recognition of exceptional performance. These awards augment the Schulich School of Medicine & Dentistry (SSMD) Dean’s Awards of Excellence. Recipients will be notified of their award in the spring of 2012.
Educators' Award of Excellence in Medical Education
Nominations:
Nominations may be submitted by any active learner currently enrolled in the Windsor Program of the SSMD or any SSMD Resident who has completed a rotation/s in Windsor during 2011-2012. Each nomination must be submitted with supporting written documentation. Please provide detailed, descriptive comments to explain why you have nominated the individual. Remember award recipients are selected solely on the information that you provide in your nomination materials.
Eligibility:
To be eligible for this award, the nominee must be an appointed faculty member, facilitator/educator, Schulich Windsor Program staff member, hospital associate or volunteer actively engaged in the Windsor Program within the 2011-2012 academic year.
How to Nominate:
Step 1: Complete the nominator and nominee information on the nomination form.
Step 2: Tell the Story! Complete the 'Description of Nominee' section by providing specific examples of how/why the nominee should be considered for the award.
Step 3: Using this form, please submit your nomination. This form must be returned (either electronically or by hard copy) to Ann Sovan, Schulich School of Medicine and Dentistry, Windsor Program, 401 Sunset Ave, Medical Education Building, Windsor, Ontario N9B 3P4 or (asovan@uwindsor.ca) before Monday February 13, 2012.

Those nominated will be adjudicated by a committee comprised of the Windsor Program Associate Dean (Chair), the Assistant Dean Faculty & Governmental Affairs and the Program Manager. Recipients will be notified of their award in the spring of 2012.
