- UCAPT Optional Self-Assessment (note: this is optional)
- UCAPT Optional Teaching Dossier or any other submission on teaching (note: this is optional)
Provided by Department Head:
Submission:
Department Head to forward to the Dean (in departmentalized faculties) and Dean to forward to the Provost and Vice-President, Academic's office in the following order, as one bundle per candidate:
- Completed cover sheet
- Completed UCAPT Rating and Evaluation Form
- Complete and accurate candidate electronic c.v.
- Completed UCAPT Record of SET scores and Student Course Rating Tables (SOST)
- UCAPT Optional Candidate Self-Assessment statement
- UCAPT Optional Teaching Dossier or any other submission on teaching, if provided by the candidate
- All performance reviews since date of hire, plus all responses
- At least three (3) letters of reference for Tenure and for Promotion to Professor considerations
- AAU PTR Criteria
