ORS Internal Form

To obtain an institutional signature on a grant application, contract, or agreement, applicants must complete the ORS Information Sheet and Checklist Form (PDF fillable form) and submit a signed copy, along with a copy of the grant application, contract, or agreement, to the Office of Research Services, five business days ahead of the sponsor's deadline.
