Medical Certificate (.pdf format)
Students who miss class due to illness need to present a Medical Certificate to the office immediately upon return to class. Students need to collect this form from the office and have it completed by a physician.
Religious Observance Form (.pdf format)
Should students wish to be excused from a class or an exam due to a religious observance (recognized religious holidays or prayer time), they need to complete the Religious Observance Form (available from the office). Students should submit such requests within the first week of classes.
