The University of Windsor accepts medical notes or certificates signed by any health care professional regulated under either the Health Professions Act (RHPA) or the Social Services Work Act or equivalent in any other provincial/state jurisdiction.
A standardized form that Faculties may wish to adopt is appended. Faculties that choose to continue with their own form are encouraged to ensure adherence to privacy regulations. For example, the form should not require a statement of diagnosis.
The Student Medical Certificate must be used by all students and completed by a physician for any test/midterm/exam that is missed due to medical reasons. It is ideal that you provide the form through Brightspace or e-mail at the beginning of your course for students.
