APPLICATION DEADLINE EXTENED TO:

The Ph.D. Applicant is required to submit two sets of Application forms:

1. An on-line Application  which is administered by Ontario University Application Centre (OUAC) and will be available October 6, 2011 for 2012 entrance. must be completed.
   A $105.00 application fee is required when submitting this Application. Please note that you do not require the Confidential Report Forms for the Faculty of Graduate Studies as the School of Social Work requires three similar Reference Forms (see below).
2. An Application to the Ph.D. of Social Work Program, School of Social Work, University of Windsor.
   The following will provide you with the information and forms required for admission to the School of Social Work Ph.D program. We are admitting students to the full-time program for the Fall of 2012. Please refer to the Ph.D Program description and admission requirements in this program of study before you begin.
