A ‘Request Form’ is to add a position and activity to the CCR database. Once a ‘Request Form’ is approved, you, the student, must sign back in to mySuccess and add the activity to your transcript. ’Request Forms’ are submitted by students, if faculty/staff would like to add an activity to the database, they should contact the CCR Program Assistant at ccr@uwindsor.ca. The Request Form can be found at the top left-hand corner of the "About the Record" page. If you have any questions please email ccr@uwindsor.ca. A Request Form can be submitted at anytime. Please note: Request Forms are subject to approval, and can be denied if certain CCR criteria is not met.
