Nursing Badge

This form is for new students only. If you require a replacement badge, please send us an email at uwincard@uwindsor.ca or visit us during regular hours.

Nursing badges are a $16 fee, payable via cash, debit, or credit when you pick up your badge.

Photo Requirements:

Full face must be clearly visible and facing forward - no profile shots, sunglasses or hats.
Photo must be in colour - passport photo style is best.
Student must be the only person in the picture.
No graduation caps, gowns, or costumes can be worn.
Photographs must have a uniform white background with no shadows.
Nothing can be touching the face such as hands, trees or graduation certificate.
The picture file must be in .jpeg format. Please name the picture file your nine-digit student number.

Monday thru Friday 9:00 am-4:00 pm (closed for lunch from 12:00-1:00 pm)

The office is closed on weekends and statutory holidays.
